How the intake form works
Completed during online booking
When you book a New Patient Exam through our online scheduling system, the intake form appears automatically as part of the booking flow — right after you select your appointment time. You will complete it before confirming your booking.
Takes about 3–5 minutes
The form asks seven short questions about your health history, the main concern bringing you in, and consent to be contacted by the clinic. All fields are straightforward — no lengthy medical history questionnaire.
Nothing to print or bring
Your responses are received directly by our scheduling system and reviewed by Dr. Mercer before your appointment. You do not need to arrive early to fill out paperwork.
